About this policy
City Beach Speech is a mobile paediatric speech pathology practice in Perth, run by Anastasia Trajcevska.
This policy explains how I collect, use, store and share personal information.
As a health service, I follow the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
What information I collect
I only collect what I need to provide speech pathology services. This may include:
Your name, phone number, email address and home address
Your child's name, date of birth, and their school or daycare
Health and developmental information about your child, including assessment results, session notes and reports
Letters or reports from other professionals, such as a GP, paediatrician or teacher
Health fund details, if you'd like help with a claim
Payment details needed to process fees
How I collect it
Most information comes straight from you. That might be through a phone call, an email, the contact form on this website, or forms you fill in before your first session.
With your consent, I may also receive information from your child's school, daycare, GP or other health professionals.
Why I collect it
To assess your child and provide therapy
To keep accurate clinical records
To book and confirm sessions, including text reminders
To send invoices and process payments
To write reports and talk with others involved in your child's care, with your permission
To reply to messages sent through this website
Who I share it with
I don't sell or rent personal information to anyone.
I only share it outside the practice when you have agreed, or when the law requires or allows it. For example:
Your child's teacher, school or daycare, when you've asked me to work with them
Your GP, paediatrician or another health professional involved in your child's care
Your health fund, if you ask me to help with a claim
Service providers that help run the practice, such as practice software, payment and website providers
I may also share information without consent if the law requires it, or if there's a serious threat to someone's life, health or safety.
How your information is stored
Records are kept securely, in password-protected systems or locked storage.
Only people who need the information to provide care can see it.
Health records are kept for as long as the law and professional guidelines require. After that, they are destroyed securely.
Information stored overseas
This website runs on Squarespace. Messages sent through the contact form may be stored on servers outside Australia, including in the United States.
Some other service providers may also store data overseas. I choose providers that take reasonable steps to keep information secure.
Using this website
When you visit, the website may collect standard technical details, such as your browser type, the pages you view and how you found the site. This is done with cookies.
It helps me understand how the site is used and keep it working well. It doesn't identify you personally.
You can turn off cookies in your browser settings. Some parts of the site may not work as expected if you do.
Anything you send through the contact form is only used to reply to you.
Seeing or correcting your information
You can ask to see the information I hold about you or your child. You can also ask me to correct anything that is wrong or out of date.
Give me a call or send a message through the contact page. I'll respond within a reasonable time, usually within 30 days.
In rare cases, the law may allow me to refuse access. If that happens, I'll explain why.
